Join WCS Healthcare Partners, a nation-wide primary care organization focused on providing exceptional care to the geriatric population. We are seeking Advanced Practice Providers for our Columbia, SC location. This position offers a unique opportunity to work in a collaborative environment, providing high-quality care with a focus on value-based outcomes.

Function

As a part of our team, you will work alongside a collaborative care team, including 1 MD, APP, Scribe, MA, and shared RN/Social Work support. You will manage 12-16 patients per day once fully ramped up, with the patient population primarily Medicare (over age 65) and up to 25% Medicaid (under age 65). Our organization utilizes the EPIC EMR system with scribe support and advanced algorithms to reduce hospitalizations. Specialty support is available through telemedicine partnerships.

Location

• Monday-Friday, 8am-5pm schedule (no nights or weekends)
• Call schedule rotated amongst market, on average 1 week every 3 months (less frequent as more clinics open)

Position Details

• Full-time permanent position with a 4-6 week orientation/training period

Responsibilities

• Provide comprehensive primary care to geriatric patients
• Collaborate with a multidisciplinary team to ensure high-quality patient outcomes
• Utilize telemedicine and EMR systems effectively
• Participate in call rotations and contribute to team-based care initiatives

Requirements

• Board Certification Required
• SC State License Required
• Training as a Family NP or Adult NP required
• Previous primary care or skilled nursing facility/nursing home experience preferred, but not required
