Geriatric RN Care Manager (Casual) - In-Home & Community
Choice in Aging
Choice in Aging is seeking a Registered Nurse Care Manager - Casual (RNCM) to support frail, low‑income elders enrolled in the MSSP Medi‑Cal Waiver. You will evaluate participants holistically, coordinate health and social services, and work within the CiA team to help clients age in place with dignity in community. The RNCM counsels and coordinates care, conducts home or remote assessments, and ensures documentation and licensing requirements are met. #J-18808-Ljbffr Choice in Aging
