Registered Nurse - Case Management - Hospital

Job Description

Job Summary:

Seeking an experienced Hospital Case Management RN to coordinate patient care plans, facilitate interdisciplinary communication, and ensure compliant transitions of care. The role requires active licensing, strong clinical judgment, and adherence to facility protocols.

Responsibilities:

• Assess patient needs and develop individualized care management plans.
• Coordinate with healthcare teams to facilitate timely discharge and transitions.
• Review patient charts for comprehensive documentation and compliance.
• Educate patients and families regarding care plans, treatments, and follow-up.
• Maintain accurate records and ensure adherence to regulatory standards.
• Comply with organizational policies, infection control, and safety protocols.

Qualifications:

• Hold a valid California RN license with no pending or temporary licenses.
• Possess a minimum of 2 years acute care case management experience.
• Certification in BLS required; prior EPIC system experience preferred.
• Demonstrate strong communication, critical thinking, and organizational skills.
• Be able to work daytime hours in a 5x8 schedule with flexibility as required.
• Maintain current flu vaccination during flu season without exemptions.
