Care Coordinator

The role of Care Coordinator involves supporting children aged 0-3 with developmental delays or disabilities and their families through a federally mandated Birth to Three Program . Responsibilities include conducting functional assessments, developing and implementing individualized, family-centered service plans, coordinating services, and facilitating team meetings. The coordinator partners with families, service providers, and community resources to promote child development, offering education, advocacy, and support. Qualifications required are a bachelor's degree in Social Work, Child Development, or related fields, with preferred experience working with families with special needs. Essential skills include strong documentation, communication, organizational abilities, and comfort working in diverse settings. Additional requirements include a valid driver’s license, reliable transportation, and crisis response skills. Work involves community-based travel, physical activity, and occasional overnight stays.
