Care Coordinator

Role Full-Time Care Coordinator supporting children ages 0-3 with developmental delays or disabilities within a federally mandated Early Intervention program. Responsibilities Partner with families, service providers, and community resources to support child development; conduct assessments; develop and implement individualized, family-centered service plans; coordinate and facilitate services; provide family education and advocacy; maintain accurate documentation; lead family-centered team meetings; collaborate with internal teams and community providers; manage a flexible schedule based on client needs; meet productivity expectations. Qualifications Bachelor’s degree in Social Work, Child Development, or related field; one year of supervised experience preferred; valid driver’s license and reliable transportation; strong organizational, communication, and documentation skills; ability to work in diverse settings; respond appropriately in crises. Work Environment & Skills Community-based role involving travel and physical activity; working in home, office, and community settings; comfort in handling crisis situations; ability to adapt to various environments.
