(LFCD) is a community development non-profit agency established in the City of Richmond in Contra Costa County in 1980. Today, Lao Family has expanded its operations and service footprint to two additional counties, including Alameda and Sacramento. LFCD’s headquarters office is in Oakland, CA. It delivers programs and services from 7 locations in 35 languages. The agency provides both community development real estate facilities and a diverse array of workforce, education, and human services that directly support predominantly low-income US-born high barrier families and individuals, refugees, immigrants, transitional age youth, seniors, and other special populations such as individuals with disabilities. Job Summary : LFCD is seeking a qualified and motivated Case Manager to support justice-involved individuals in obtaining and retaining meaningful employment as well as manage participants under the California Advancing and Innovating Medical (Cal-AIM) Initiative, which provides comprehensive care management to address both clinical and non-clinical needs. This position serves as the primary point of contact for participants and is responsible for developing relationships with local employers, training providers, community-based organizations, and internal LFCD departments, particularly working with Workforce Innovation Opportunity Act (WIOA) program staff and America’s Job Center of California (AJCC) Business Centers. The Case Manager will recruit and enroll justice-involved individuals, connect participants with fair-chance employers, provide employment readiness services, career coaching, and comprehensive case management support. This position requires travel and rotating assignments among multiple office locations as needed. The Case Manager reports directly to the Program Supervisor. Roles and Responsibilities: Conduct community outreach and recruitment activities to promote program services, including attending resource fairs, networking events, and community meetings to recruit justice-involved individuals. Develop and distribute program flyers, outreach materials, and promotional resources. Facilitate job readiness workshops, program orientations, interviewing skills sessions, and individual and group career counseling. Assist participants with resume development, cover letters, job search strategies, and individualized employment plans. Provide labor market information, job referrals, job club activities, and maintain participant attendance records. Develop and maintain relationships with employers, training providers, and community partners. Manage existing employer partnerships and identify new business opportunities to expand employment opportunities for participants. Conduct follow-up surveys with employers and participants to assess job satisfaction and employment retention. Provide comprehensive case management services, including participant intake, eligibility determination, enrollment, assessment, and collection of required documentation. Develop and monitor participant My Action Plans (MAPs), identify career pathways and goals, make appropriate referrals, and provide ongoing support throughout program participation. Deliver intensive employment and retention services, including job placement assistance, follow-up services, and 180-day job retention support to ensure successful employment outcomes. Coordinate supportive services and referrals, including counseling and mentoring services, mental health resources, substance abuse treatment programs, transportation assistance, and other services that address barriers to employment. Maintain accurate and timely participant records, including case notes, daily activity logs, assessments, service documentation, and data entry into the Cal E-Grants Portal and other required tracking systems. Conduct participant site visits, monitor job retention progress, coordinate supportive services, and prepare monthly program activity and performance reports for the Program Supervisor. Develop and maintain detailed knowledge of programs such as WIOA and CalAIM Program, leveraging partnerships to enhance participant outcomes. Demonstrate knowledge of and commitment to LFCD's mission, vision, core values, and strategic plan. Participate in agency initiatives and continuous quality improvement efforts to enhance program effectiveness and service delivery. Support the agency's Commission on Accreditation of Rehabilitation Facilities (CARF) accreditation efforts by ensuring compliance with applicable standards, participating in accreditation activities, and contributing to the achievement of agency goals and objectives. Perform other duties and special projects as assigned by the Program Supervisor Requirements and Qualifications: Bachelor's degree in Human Services, Social Work, Psychology, Counseling, Workforce Development, Public Administration, or a related field required. An equivalent combination of education and experience may be considered. Minimum of two (2) years of experience providing direct employment services, career coaching, workforce development, case management, or related services. Self-motivated and able to work independently with minimal supervision. Demonstrated ability to prioritize tasks, manage multiple responsibilities, and meet deadlines. Excellent verbal and written communication, community relations, networking, facilitation, and public speaking skills. Demonstrated ability to work effectively and respectfully with individuals and families from diverse cultural, racial, ethnic, socioeconomic, religious, gender, age, disability, and sexual orientation backgrounds. Knowledge of workforce development programs and the ability to understand, interpret, explain, and apply complex local, state, and federal regulations, policies, procedures, and program requirements. Ability to establish and maintain effective working relationships with participants, employers, community partners, service providers, and agency staff. Ability to travel throughout the service area to meet with employers, service providers, and participants. Must be available to work flexible hours, including occasional evenings and weekends, as required by program activities. Proficient in Microsoft Office Suite (Word, Excel, PowerPoint), database management systems, data entry, and internet applications.
