Occupational Therapist (OT) – PRN
Location: Silver Memories Health Care – Versailles, IN

Silver Memories Health Care is seeking a compassionate and dedicated PRN Occupational Therapist (OT) to join our rehabilitation team in Versailles, Indiana. This flexible, as-needed position offers the opportunity to make a meaningful impact on residents’ quality of life by helping them achieve greater independence and function in their daily activities.

What You'll Do:

• Evaluate residents and develop individualized occupational therapy treatment plans based on clinical needs and goals

• Provide skilled occupational therapy services in accordance with physician orders, professional standards, and facility policies

• Assess residents’ functional abilities and recommend adaptive equipment, environmental modifications, and therapeutic interventions

• Collaborate with interdisciplinary team members to support optimal resident outcomes

• Complete timely and accurate documentation in accordance with regulatory and company requirements

• Participate in resident care planning and discharge planning as appropriate

What You'll Bring:

• Degree in Occupational Therapy from an accredited program

• Current Occupational Therapist license in the State of Indiana

• Must be 18 years of age or older

• Strong clinical, communication, and documentation skills

• Commitment to continuing education and maintaining licensure requirements
