Work Environment

This position requires working onsite, primarily in home care settings, where you will provide care directly to patients in their homes or other community locations. Key Responsibilities

• Perform comprehensive patient assessments, including obtaining medical histories and conducting physical examinations to develop an in-depth understanding of each patient's health status.
• Diagnose and manage acute and chronic health conditions, creating tailored treatment plans based on individual patient assessments.
• Order, interpret, and communicate results of diagnostic tests, such as blood work and imaging studies, guiding clinical decisions for optimal patient outcomes.
• Prescribe medications and therapeutic interventions, ensuring adherence to clinical guidelines and monitoring for effectiveness and side effects.
• Provide ongoing management of health conditions, adjusting treatment plans as necessary and educating patients on health maintenance and disease prevention strategies.
• Work collaboratively with physicians, specialists, and other healthcare professionals to ensure a holistic approach to patient care.
• Facilitate referrals to specialty services when advanced care is required, ensuring continuity in patient treatment.
• Engage in medical procedures, such as suturing, administering vaccinations, and managing wound care, directly contributing to patient health improvement.
• Participate in clinical reviews and contribute to staff performance evaluations, mentoring junior staff and students in best clinical practices.
• Stay current with healthcare advancements through ongoing education and involvement in research projects aimed at improving evidence-based practices.

Patient Population / Client Focus

The primary focus of this role is to serve individuals within the Salisbury community who may require home-based health services due to mobility issues, chronic illnesses, or post-operative care. Qualifications

• Bachelor's Degree in Nursing (BSN) from an accredited institution, followed by a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) specializing as a Nurse Practitioner.
• Active and valid state Advanced Practice Registered Nurse (APRN) license required for practice.
• Certification as a Nurse Practitioner in a relevant specialty (e.g., Family, Adult-Gerontology, or Pediatric Nurse Practitioner).
• Proven ability to perform precise assessments and create comprehensive treatment plans effectively.
• Strong interpersonal skills to communicate effectively with patients, their families, and healthcare colleagues.

Preferred Experience

Compensation and Benefits

This position offers a competitive salary range of $93,600 to $140,000 annually, commensurate with experience and qualifications.
