Work Environment

The role is situated in a supportive and collaborative onsite environment at an assisted living facility in Albany, NY. Working during the hours of Monday to Friday, 9 am to 5 pm, you will engage with a multidisciplinary team committed to delivering exceptional care. Key Responsibilities

As a Nurse Practitioner, you will be responsible for:

• Conducting comprehensive health assessments to evaluate patients' physical and mental well-being.
• Managing acute medical situations and exacerbations of chronic conditions, employing diagnostics and interventions tailored to individual patient needs.
• Overseeing the care of members during short-term sub-acute rehabilitation episodes, ensuring smooth transitions and continuity of care.
• Maintaining accurate and complete documentation of patient conditions, utilizing ICD-10 coding for precise health record-keeping.
• Collaborating with an interdisciplinary care team to coordinate treatment plans and ensure holistic patient care.
• Engaging in advanced illness and care planning discussions, guiding patients and families through critical health-related choices.
• Providing education and counseling to patients and their caregivers to promote health literacy and self-management of health conditions.

Patient Population / Client Focus

This role primarily focuses on residents of the assisted living facility, who often present with complex medical, behavioral, and social needs. You will provide care for patients with chronic illnesses, manage acute health issues, and support individuals as they navigate advanced illness and end-of-life discussions. Your clinical expertise will be crucial in fostering improved health outcomes and enhanced quality of life for our diverse patient population.

Qualifications

To qualify for this role, candidates should possess:

• Current certification as a Nurse Practitioner.
• An active nursing license in New York State.
• A Master's degree in Nursing obtained from an accredited program.
• A minimum of three years of clinical experience, particularly in settings that involve intensive patient care.

Preferred Experience

While the minimum qualifications are essential, the following experiences are preferred:

• Clinical background in long-term care, skilled nursing facilities, assisted living, acute care, or emergency/urgent care settings.
