Community Case Manager

Responsibilities Coordinate services for adults with developmental disabilities at their homes, work, and with community supports. Work collaboratively with families, spouses, providers, and state agencies. Maintain independent, self‑motivated work style and multitask in a fast‑paced environment. Support a very supportive team for assistance and guidance. Qualifications BA in psychology, human services, social work, special education, rehabilitation or a closely related field. Must own a dependable automobile with comprehensive insurance coverage; mileage reimbursement provided. Benefits Comprehensive medical, dental, prescription, and vision coverage.
