Primary Care Physician Lead

Humana's Primary Care Organization is a provider of senior-focused, value-based care, with more than 340 centers across 15 states under the CenterWell and Conviva brands. This is a full-time, on-site, patient-facing role. You report to the Market Chief Medical Officer (CMO), or Associate Medical Director (AMD), and are responsible for both direct patient care and clinical leadership for the Anderson, SC clinic. About The Role You will provide primary care for senior patients in a value-based care model while leading a team of clinicians to lead quality, outcomes, and patient experience. You will balance patient care with leadership responsibilities, supporting clinicians, improving performance, and advancing a patient-centered model of care. What You'll Do Clinical Care You will provide primary care for senior patients, including those with complex and chronic conditions You will see patients and manage ongoing care, typically spending 50–80% of your time in direct patient care You will complete accurate documentation to support care and outcomes You will manage a clinical inbox, including labs, messages, and follow-ups You will coordinate care with specialists and interdisciplinary care teams You will apply Medicare risk adjustment principles through accurate condition documentation Clinical Leadership You will lead a team of Physicians, Nurse Practitioners, and Physician Assistants You will coach clinicians using data, performance dashboards, and patient outcomes You will participate in hiring, onboarding, and retention of clinical staff You will provide performance feedback, complete evaluations, and support professional growth You will foster a culture of collaboration, accountability, and continuous improvement Quality & Performance You will improve performance in patient experience, quality, and clinical outcomes You will monitor and improve important metrics such as: Net Promoter Score (NPS) HEDIS and Medicare Stars measures Total cost of care and avoidable utilization You will conduct chart reviews to identify care opportunities and guide improvement Operational Partnership (Dyad Model) You will partner with operational leaders to align on clinical and business goals You will collaborate on provider schedules, patient access, and care delivery strategies You will support performance management, team engagement, and clinic operations You will contribute to consistent communication and aligned decision-making across teams What You Can Expect You will work in a value-based model focused on outcomes—not volume You will balance direct patient care with meaningful leadership impact You will lead within a team-based care environment focused on whole-person care You will mentor clinicians and shape clinical performance You will benefit from a supportive culture with PTO, CME, and manageable call expectations Use your skills to make an impact Required Qualifications MD or DO from an accredited program Board Certified in Family Medicine, Internal Medicine, or Geriatrics Active, unrestricted medical license (or ability to obtain) in the state of practice 8+ years of clinical experience 2+ years of leadership or project leadership experience Preferred Qualifications Experience managing Medicare Advantage patient panels Experience in value-based care or coordinated care environments Experience supervising Advanced Practice Providers (NPs/PAs) Active, unrestricted DEA license Medicare and Medicaid provider enrollment You will receive a compensation package that may include: Sign-on bonus and relocation support (if applicable) Medical, dental, and vision insurance Paid time off (PTO) and CME support 401(k) with employer match Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $292,000 - $341,500 per year This job is eligible for a bonus incentive plan.
