Location: Fremont, California
Compensation: $60–$65/hour
Schedule: 35 hours/week | 5 days/week
Weekend Rotation: Required

We are seeking a compassionate and motivated Occupational Therapist (OT) to join a 126-bed skilled nursing facility in Fremont, California . The patient population includes Kaiser patients, short-term rehabilitation patients, hip and knee replacement patients, and individuals recovering from neurological conditions . You'll have the opportunity to work with a diverse range of clinical presentations while collaborating with an experienced interdisciplinary team.

• Perform comprehensive occupational therapy evaluations and assessments

• Develop individualized treatment plans based on each patient's functional needs and goals

• Provide skilled OT interventions to improve independence with activities of daily living and functional mobility

• Treat patients recovering from orthopedic procedures, including hip and knee replacements

• Provide therapy for patients recovering from neurological conditions and other complex medical diagnoses

• Monitor patient progress and modify treatment plans as clinically appropriate

• Educate patients and caregivers on therapeutic techniques, safety strategies, and continued progress toward independence

• Collaborate with physical therapists, nursing staff, physicians, and other members of the interdisciplinary team

• Participate in care planning and discharge planning

• Maintain accurate, timely, and compliant clinical documentation

• Communicate effectively with patients, families, caregivers, and other healthcare professionals

• Follow applicable clinical standards, facility policies, and regulatory requirements

• Graduate of an accredited Occupational Therapy program

• Current California Occupational Therapist license or eligibility for licensure

• Strong clinical reasoning and communication skills

• Commitment to patient-centered care

• Ability to work effectively within an interdisciplinary team

• Strong organizational and documentation skills

• SNF or rehabilitation experience is a plus

The $60–$65/hour range is intended for Occupational Therapists with 1+ years of professional experience .
