Registered Nurse - Hospice

Job Description

Job Summary:
Provide compassionate, patient-centered hospice nursing care in various home settings, ensuring comfort and dignity for patients facing life-limiting illnesses. Lead interdisciplinary teams to develop and implement personalized care plans that meet each patient's goals.

Responsibilities:

• Assess patient health conditions and identify symptom management needs through comprehensive evaluations.
• Educate patients and caregivers on disease processes, care strategies, and symptom control to foster confidence and independence.
• Collaborate with interdisciplinary team members to develop and execute individualized care plans.
• Administer medications, treatments, and interventions according to established protocols and patient needs.
• Document patient progress, treatment responses, and care activities accurately within the electronic medical record.
• Provide emotional support and guidance to patients and families during challenging end-of-life journeys.

Qualifications:

• Valid RN license with current state licensure.
• Proven clinical experience in hospice, palliative, or related healthcare settings.
• Strong assessment, clinical reasoning, and critical-thinking skills.
• Excellent communication abilities for patient education and team collaboration.
• Compassionate, empathetic, and able to provide emotional support in sensitive situations.
• Ability to work independently and manage a caseload effectively.
