Licensed Vocational Nurse (LVN)
Job Summary:

Responsibilities:
Provide direct patient care to clients, including wound care, medication administration, and vital sign monitoring
Develop and implement individualized care plans in collaboration with the healthcare team
Conduct routine assessments and report findings to the physician or nurse practitioner
Administer medications and treatments as prescribed by physician or nurse practitioner
Collaborate with other healthcare professionals to ensure comprehensive care and support
Maintain accurate and up-to-date records of patient care and treatment
Participate in quality improvement initiatives to improve patient outcomes
