Family Practice/Primary Care Physician Assistant

Position Overview

Full-time outpatient NP role
Medicare patient population
Independent practice with physician collaboration
Reports to Center Medical Director

Responsibilities

Provide routine and acute primary care visits
Conduct annual wellness visits and HRAs
Coordinate care with specialists and care teams
Support population health and quality initiatives
Patient education and care planning

Requirements

MSN, DNP, or NP Certificate Program
Board certified NP
Active NP license and DEA
Must be bi-lingual English / Spanish
2 years of primary care outpatient experience

Preferred

HCC / ICD-10 / HRA familiarity
