Case Manager, Registered Nurse

CVS Health is seeking a Registered Nurse Case Manager to coordinate holistic care for diverse patient populations. In this role, you’ll assess clinical and social needs, create person-centered care plans, and help close gaps in care across providers, benefits, and community resources. You’ll deliver telephonic and virtual outreach, educate patients and caregivers, and support safe transitions to reduce avoidable ER visits and hospitalizations. You’ll collaborate with an interdisciplinary team while leveraging CVS Health’s tools and training to grow your career in a purpose-driven, inclusive environment.
