Case Manager, Registered Nurse

CVS Health seeks a field-based RN Case Manager to support members in the community. In this role, you’ll assess clinical and social needs, develop person-centered care plans, and coordinate services across providers, payers, and community resources. You will conduct home and community visits, provide education on chronic disease and medications, and remove barriers like transportation or housing. Leveraging CVS Health’s integrated services, you’ll focus on improving outcomes, reducing avoidable hospital use, and supporting members and caregivers on their path to better health.
