Job Requisition: HOSPI001848 Employment Type: Full-time Shift: Days Hours Per Week: 40-hour/week

Hiring range: Salary $31.62/hr - $38.59/hr The actual compensation for this position will be determined based on experience and other factors permitted by law.

Responsibilities:

• Reviews all external referrals which include (but are not limited to) Hospitals, other Nursing Homes, Adult Homes and home referrals.
• Screens for appropriateness for Wilkinson's RHCF based on census
• Closely communicates with Nursing staff and the Admissions Coordinator at the nursing home regarding admissions.
• Communicates and provides information to Case Management and hospital patients, educating them about the Adult Day Care (ADHC) as a service that can be offered to eligible patients returning home.

Education, Training & Experience:

High school diploma/GED with a minimum of three (3) years of long term care or hospital setting experience required. Minimum of one (1) year experience required if applicant is RN, LPN or LMSW

• This position requires travel to various capital district hospitals & other facilities at a minimum of 95%
• Experience with referral software (EPIC and Careport) preferred but not required
• Valid drivers license required. Licensed RN, LPN or LMSW preferred.
