The Children & Family Services Department at Community Connections, Inc is seeking an Early Intervention Service Coordinator to provide case management to children birth to three years old with developmental challenges and their families. An ideal candidate will possess computer and database experience, but most importantly, a desire to work with families in their homes and via computer or phone to ensure that their children reach their full potential. This position requires someone who understands child development, can multi-task, be a good listener, and help families process their concerns that their child may or does have a developmental delay. We are willing to train an individual who pays close attention to detail, is self-motivated, and meets deadlines. Bi-lingual and multicultural experience are ideal but not required. Location: Hybrid Position based out of Pagosa Springs, CO
Hourly Rate: $22.05
Job Summary: This role provides Service Coordination for the EI Program supporting children birth – 3yo with developmental challenges and their families. EI families are located primarily in Archuleta County, with occasional families in Eastern La Plata County. Communication is conducted in-person and via phone/videoconferencing.
Job duties:
- Be the initial communication with referred families
- Scheduling and coordinating evaluations of referred children
- Scheduling and facilitating Individualized Family Service Plans
- Working with families to understand their needs and providing referrals to assist in meeting them
- Checking in at least monthly
- Working with providers to ensure expected service delivery
- Ensuring all documentation is entered accurately in the state database and Community Connections Electronic Health Records System
- Must be willing to complete online trainings required by the state
- Service Coordinator caseload is approximately 15-25 families
Responsibilities:
- Be responsive to individuals needs of clients and families
- Participate in state-wide Early Intervention Service Coordination Training, IFSP Training and other Early Intervention trainings
- Communicate with parents referred to Early Intervention and be able to explain the Early Intervention philosophy and model
- Follow HIPAA and other procedural safeguards to ensure confidentiality for families and providers
- Carry a case load of children as assigned by the Director of Children and Family Services
- Work with Early Intervention evaluators and families to schedule evaluations of referred and enrolled children
- Facilitate IFSP meetings for families enrolled in EI
- Monitor implementation of the IFSP
- Assure all procedures and related documentation are completed for all families in accordance with state timeline requirements throughout the program (including referral, evaluation, service planning and delivery and transition).
- Participate in Trans-disciplinary teaming meetings with providers
- Work directly with supervisor to assist in the development and ongoing changes of policies and procedures
- Demonstrate knowledge about resources in the community and share this with families as appropriate
- Assist families with transition when their child leaves Early Intervention
- Assist in carrying out Federal Assurances
- Other related duties to support the Early Intervention program

- Satisfactory completion of all background checks and references, including Motor Vehicle Record is required***
Qualifications: Must hold a B.A or Master's in a related field or at least 5 years' experience working in the human services field with families, children and/or adults, or some combination of education and experience appropriate to the requirements of the position. Must demonstrate proficiency in Microsoft Word, using databases, with skills in general computing and Windows operation.
Physical Demands of the Job: These essential tasks may be performed with or without accommodations or assistive technology. Employees in this position may spend significant hours on the computer writing and corresponding. The employee must be able to read text on the computer and paper documents. Communicating with clients via email, letters and telephone are required. An employee in this position will use a laptop computer and may also have documents and files that they will need to transport within and outside the office. The employee will be required to travel to meet families in their homes, travel to meetings, training, and other community activities within the region of Southwest Colorado, with the possibility of occasional travel within Colorado or out of state to training or conferences.
