We CARE for our patients like they are our own FAMILY.SCHEDULE: Monday - Friday, 8:00am - 4:30pm; On-Call Every 20th Weekend; 1 Holiday Every 2 Years.COMPANY: Adena HospiceThe Registered Nurse Case Manager (RNCM) - Hospice assumes ultimate accountability and leadership for the assessment of the patient condition and plan of care. The RNCM provides case management, clinical care and is responsible for coordination all disciplines involved in providing quality, cost-effective and billable care through direct care and supervision of care under the direction of the physician. ASHN adopts the RN scope of practice as outlined by the state for its licensed RN personnel.QualificationsOne year of experience as a RN in an acute care or home care setting is required.Hospice experience is preferred.Maintains active RN license in state of employmentUnderstands and practices agency policies and proceduresMaintains a professional appearance as a representative of the companyAbility to remain calm, have patience and be accommodating. Compassionate and caring while working with patients and families.Knowledgeable on nursing best practices. Ability to make appropriate nursing judgments. Ability to identify a situation and handle it with the best possible solution. Detail-oriented and observant.Disciplined style of work ethic. Ability to prioritize and be timely.
