About Us The Caring Place (@Miami Rescue Mission & Broward Outreach Centers) is a leading provider of services to the homeless and needy of South Florida. We are looking for like‑minded, dynamic individuals to come and join our team. The Caring Place provides emergency shelter, residential programs, job training, education & computer literacy classes, healthcare, transitional housing, permanent housing, employment opportunities for homeless men, women and children as well as programs for “at‑risk” youth. Job Summary The Lead Case Manager (LCM) reports to the Behavioral Health Manager (BHM) and provides case management services, crisis intervention, support and referral services for clients, while assisting the BHM with day‑to‑day operational tasks. The LCM will also compile timely outcome reports per program guidelines and requirements and perform related duties as needed. Core Duties / Responsibilities Provides assistance to the Emergency Services Case Managers team with a professional demeanor and cooperativeness. Communicates effectively with the Behavioral Health Manager . Participates in staff file peer reviews of care plans and client files, performing follow‑up to determine quantity and quality of services provided and status of client cases. Attends staff meetings for case review and consultation on difficult or unusual cases. Collaborates with Behavioral Health to ensure cohesiveness and meets regularly for crisis assessment team meetings weekly. Compiles timely monthly outcome reports per program guidelines and requirements and performs additional duties as needed for the Behavioral Health Manager . Conducts needs assessments with all assigned clients, verifies data collected in intake, and makes recommendations for services. Ensures that all data entry information, initial care plans, case notes and follow‑ups are entered into the service point database daily. Monitors all assigned cases, keeping files up to date and in accordance with established policies, procedures and funding requirements. Documents all outcome measures for reporting purposes and ensures data is entered within three (3) business days upon initial referral or discharge of the client. Prepares accurate incident reports and keeps the manager informed in a timely manner. Establishes and maintains effective working relationships with clients while maintaining appropriate boundaries. Ensures that all clients are treated with respect and dignity and maintains client confidentiality. Helps clients to self‑identify destructive attitudes and behavior patterns by increasing awareness of personal accountability. Secures cooperation of social service agencies and local community organizations to meet client needs, advocates for the development and/or maintenance of services, refers clients and assists with applications to Food Stamp office, Department of Children & Families (DCF), Department of Motor Vehicles (DMV), Social Security Administration (SSA), Department of Health/Vital Statistics (DHS – birth certificates), etc. Will lead and/or participate in devotions, prayers or other ministry activities such as outreach events. Must agree with and sign The Caring Place Statement of Faith. Performs other related duties as assigned. Education / Experience Requirements Requires a high school diploma or GED; addictions training and/or certification and/or a bachelor’s degree in social work preferred, Certified Addictions Professional (CAP) a plus. Must have at least three (3) years of demonstrated experience working with the homeless population. Must be able to compile timely monthly outcome reports per program guidelines and requirements, service point updates and other reports as assigned. Must possess excellent communication skills and serve as the liaison between Emergency Services Case Managers and the Behavioral Health Manager . Must exercise independent judgment within the framework of approved case management techniques, existing laws and agency guidelines. Must recognize symptoms of common mental, physical, co‑occurring, alcohol/substance abuse addictions to make proper assessment for care plans, referrals and linkage with excellent follow‑up skills. Must possess knowledge of local community agencies, programs and resources available to clients. Must be able to establish and maintain effective working relationships with clients and staff of diverse cultural and linguistic backgrounds, regardless of race, religion, age, sex, disability, political affiliation and sexual orientation. Must react quickly and calmly in an emergency. Must be computer literate and have experience with MS Office Suite (Word, Excel and Outlook). Must understand, speak, read and write English and carry out oral and written instructions. Physical Requirements Must be able to sit for extended periods and lift or pull objects weighing up to 25 pounds. Must be able to see, hear and speak to interact with staff and the public.
