Description

American Traveler is hiring an RN Case Manager for a Discharge Planning role requiring an active MA RN license and at least 2 years of case management experience.

Details

• Case Management department in an acute care hospital setting
• Discharge planning focus within Case Management
• 40 hours per week, day shift Monday through Friday
• Schedule may consist of 4x10 or 5x8 shifts depending on operational need
• Shift hours are 07:30-18:00
• 13-week assignment
• Uses Epic EMR charting system

Requirements

• Active MA RN license required (no pending or conditional licenses accepted)
• Minimum 2 years of case management experience required
• At least 6 months of discharge planning case management experience in an acute care hospital required
• Experience in a large organization or teaching hospital with 500+ beds strongly preferred
• AHA-only certifications accepted
• Flu vaccine required with no religious or medical exemptions allowed
• Two professional references required for consideration

Additional Information

• Responsible for discharge planning duties within the case management team
• Local candidates must reside within 60 miles of Boston, MA to be considered for the local rate
