Care Coordinator

Description Career Details: We are seeking a passionate Care Coordinator to assist residents/clients with appropriate linkages and referrals to community resources. Ensure services are delivered in accordance with the mission and goals of the agency and correspond with agency/governing body policies, procedures, and contracts. Employees will engage with clients on a consistent basis and utilize cognitive behavioral techniques in order to foster and promote healthy decision making and positive interpersonal skills. Requirements Education: Associate's or Bachelor's degree in human services, social work, criminal justice, behavioral sciences, or other related field.
MUST HAVE A VALID DRIVER'S LICENSE
Experience: Associate's degree: minimum three years of relevant prior work experience Bachelor's Degree: no minimum experience required. Requires: VALID DRIVER'S LICENSE Computer Skills: Skill in use of personal computer and related software (e.g., Electronic records, Microsoft Word, Excel); use of office equipment (e.g., copier, fax, printer).
