Registered Nurse Care Coordinator

Infinite Medical P.C. in Columbus, OH is seeking a licensed Registered Nurse to serve as Nursing Care Coordinator on-site. The role focuses on administrative review of chart documentation to ensure CMS compliance and continuity of care for frail residents. You will collaborate with HR, Operations, and Clinical teams; maintain proficiency with PCC, MatrixCare, SigmaCare, Emsow, Reliable/Visual, Yardi-One, and Google Workspace; and participate in mandatory meetings with a daytime schedule #J-18808-Ljbffr
