Bilingual Health Home Care Manager for Medicaid (NYC)

Doral Health & Wellness in New York City is seeking a Health Home Care Manager (Spanish-speaking) to coordinate person-centered services for Medicaid-enrolled clients with chronic medical, behavioral health, or substance use needs. This hybrid role includes office work and in-person community and home visits across NYC.
