MAKE AN IMPACT

As our Clinical Director | Care Concierge , you’ll lead clinical operations while serving as a trusted advocate and care coordinator for our residents and families. Through our partnership with Centered Care , you’ll use proactive care practices, technology, and clinical insight to identify needs early and help ensure residents receive the right support at the right time. You’ll have the opportunity to: Lead and develop a high-performing clinical team. Oversee individualized resident care and service plans. Build strong relationships with residents, families, physicians, and healthcare partners. Coordinate care with physicians, specialists, pharmacies, labs, home health, hospice, and therapy providers. Lead interdisciplinary team meetings and care conferences. Oversee medication management, clinical documentation, and quality initiatives. Coach and educate caregivers and clinical team members. Support dementia care, resident transitions, and complex healthcare needs. Serve as a clinical leader during urgent situations and participate in the director on-call rotation.

WHAT YOU’LL BRING

Active, unrestricted Arizona RN, LPN, or LVN license . 3+ years of nursing experience in assisted living, memory care, skilled nursing, home health, or comparable senior care. 1+ year of clinical leadership or supervisory experience. 1+ year of hands-on dementia care and medication administration experience. Experience with EHR documentation and eMAR systems. Current CPR and First Aid certification. Valid Arizona fingerprint clearance card or ability to obtain one within required timelines. Valid driver's license and access to a private vehicle for business use. Preferred: Experience coordinating care across healthcare providers, behavioral health or dementia care experience, care management technology, Arizona assisted living regulations, or Arizona Assisted Living Facility Manager certification.
