Case Worker II - Infectious Diseases

Position Summary Responsible for assessing, planning, coordinating and evaluating the care of patients. Linkage to Care Case Management for adults newly diagnosed with HIV. Scheduled Hours 40 hours per week. Primary Duties & Responsibilities Conducts a comprehensive sensitive assessment with the client and family/support network as appropriate, in order to develop a plan of care to meet identified needs. Arranges and coordinates service referrals for the client/family as needed; provides ongoing monitoring of the activities implemented to carry out the plan of care. Refers clients/families to public benefits programs as eligible. Provides regular contact with enrolled clients and documents all contacts and service coordination activities in client records as required by program guidelines. Educates clients/families about the basic medical and psychosocial aspects of disease in order to optimize adherence to care. Provides proactive follow up, tracking symptoms and treatment responses. Adheres to all applicable state laws and the department’s rules and regulations regarding programs. Stays abreast of new resources, programs and developments. Participates in continuous training and education, including attendance at staff meeting and community meetings as assigned. Education Master’s degree or combination of education and/or experience may substitute for minimum education. Certifications & Professional Licenses Basic Life Support – American Heart Association Basic Life Support – American Red Cross Basic Life Support certification must be obtained within one month of hire date. Online BLS certifications without a skills assessment component do not meet the BLS requirements. Work Experience No specific work experience is required for this position.
