Hospice RN Case Manager
Manageable caseload (intentionally maintained around 15 patients) Dedicated Admissions Nurse, LPNs & On-Call Nurse support Defined territory to reduce drive time Structured weekday schedule Clinician-led leadership that understands hospice You are not expected to carry the workload alone.
Your Role
As an RN Case Manager, you are the clinical lead for your assigned patients supported by a strong interdisciplinary team. You will:
- Conduct comprehensive nursing assessments (with Admissions Nurse support)
- Develop and manage individualized plans of care
- Collaborate with physicians, LPNs, social workers, chaplains & aides
- Educate families on symptom management and disease progression
- Participate in IDT meetings
- Maintain timely, compliant documentation

Active New Jersey RN License Hospice or home health experience strongly preferred Strong clinical judgment & communication skills Valid driver's license & reliable transportation
