Our HHAs provide care to medically fragile children and adults with disabilities, ensuring they receive the highest quality care in the place they feel most comfortable—their home.

Position Overview

As a Home Health Aide you will provide unskilled services tailored to each client’s needs, following individualized care plans all in a comfortable home environment.

Key Responsibilities

Deliver direct, one-on-one nursing care in the patient’s home
Follow individualized plans of care
Monitor patient condition and document progress
Monitor for safety
Communicate changes with clinical care team
Maintain accurate and timely clinical documentation

Qualifications

Proof of training OR we can also train you if you do not have training)
Ability to pass a background check and drug screening
