Job Opportunity Position: Case Management RN Start Date: ASAP Contract Length: 13 Weeks Shift: Day 5x8-Hour (07:30 - 16:00) Location: Seattle, WA Job Description We are seeking a qualified Case Management RN to join our team for a 13-week contract. The successful candidate will be responsible for providing comprehensive case management services, including discharge planning and care coordination for patients across various areas of the healthcare facility. Key Responsibilities
- Provide care coordination and discharge planning for patients in an acute care setting.
- Work collaboratively with multidisciplinary teams to ensure optimal patient outcomes.
- Float between different units within the hospital, including Colby and Pacific campuses.
- Adhere to patient care protocols and coordinate with social services and other support staff.
- Experience: Minimum of 1 year of acute care experience and discharge planning/care coordination experience required.
- Strong preference for candidates with diverse case management experience, including Med Surg and Emergency Department.
- Current BLS certification and valid Washington State nursing license required.
- Willingness to work every other weekend is mandatory.
- Excellent communication skills and ability to work in a fast-paced environment.
- Guarantee: 72 hours every two weeks.
- Patient Demographics: Experience with a diverse patient population, including neonates, geriatrics, and adults.
- Charting System: Familiarity with EPIC is required.
- Must commit to working 3 out of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day).
