Lead Navigator

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency serving communities throughout Arizona. Lead Navigator Seasonal role, active during peak heat periods from May to October. Potential for continued employment if suitable roles become available within the agency. Job Summary A Lead Navigator is someone in recovery from AOD use and/or GMH for an appropriate period (≥1 year) and with at least 90 days experience as a Navigator or Peer Support Specialist. The Lead Navigator works with individuals who are homeless, providing outreach and supportive services to assist members with ending homelessness. The Lead Navigator serves as the bridge for the patient, coordinating and monitoring R BH A and Non-RBHA community‑based services and facilitating delivery by supporting the individual. Responsibilities Conduct outreach, engagement, case management, and ongoing wrap‑around supportive services to the individual. Identify individuals with the highest acuity and greatest vulnerabilities. Coordinate care with community providers and stakeholders. Serve as a mentor to Navigator II to onboard, guide, and help them fulfill program requirements. Master processes: coordinated entry, housing referral, leasing up, wrap‑around services, and discharge. Collaborate with multidisciplinary team including Patient Care Techs, Registered Nurses, Counselors, and Clinical Leads. Qualifications & Requirements High school diploma or GED required. Associates degree (or higher) in a field related to behavioral health preferred. 1–3 years of full‑time health‑care related work experience, or equivalent combination of education and experience. Current AZ Driver's License. Clear 3‑year (39‑month) motor vehicle record. Background: Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures. Arizona Fingerprint Clearance card, site specific.
