The setting could be a patient’s private residence or in a patient’s room in an independent living facility. This role emphasizes evaluation, treatment, and education to promote patient independence and producti­vity while minimizing pathology. Assess, develop, and deliver individualized physical therapy programs Make referrals to other disciplines to address comprehensive patient needs Implement therapy plans to restore, reinforce, and enhance performance Instruct and train patients and caregivers in essential skills and functions Promote activities that support health and safety Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel as required Adhere to treatment schedules and documentation, including billing requirements Attend patient care, staffing, and other related meetings Consult with the rehabilitation team to select appropriate therapeutic regimens Adapt programs and plans of care based on individual patient needs Enforce a safe environment and manage safety hazards Order, maintain, and report on necessary supplies and equipment Provide high‑quality patient service in a caring and compassionate atmosphere Screen, examine, evaluate patients, and develop treatment plans with appropriate goals Implement therapeutic techniques across multiple modalities Incorporate health and wellness programming into treatment plans Perform home and community mobility assessments, providing safe mobility recommendations Perform routine re-examinations, modify progress plans as needed Carry out other related duties as required Select Interventions Therapeutic exercise (aerobic capacity, balance, agility, body mechanics, postural stabilization, flexibility, gait, neuromotor development, relaxation, strength, power, endurance) Functional training Manual therapy Airway clearance techniques Integumentary repair and protection Electrotherapeutic, physical agent, and mechanical modalities Health and wellness education (chronic disease management) Falls assessment and interventions to reduce fall risk Case management in the home and community Design, fabricate, fit, and apply adaptive devices (orthotics, prosthetics) and train patients in their use Select activities suited to the individual’s physical, mental, and cognitive capacity Instruct patients in mobility skills, evaluate progress, attitudes, and behavior related to rehabilitative potential Qualifications Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility Master’s degree in Physical Therapy, or Master’s in a related field with a Bachelor’s in Physical Therapy, or Bachelor’s in Physical Therapy with 5 years of progressively responsible PT experience Degree earned outside the U.S. evaluated as equivalent to a U.S. Physical Therapy degree Valid driver’s license or reliable, independent transportation to patients’ residences Posted Salary Range: USD $87,802.00 – USD $101,565.00 /yr.
