Minimum Qualifications

• Associate degree in Physical Therapy from an accredited educational program.
• Active State License is Required to Start the Assignment
• BLS Certification May Be Required from AHA or ARC
• Home health experience required.
• PTA certification required.

Assignment Details

• Schedule: Monday through Thursday, 8:00 AM to 4:30 PM.
• 40 hours per week.
• Productivity expectation: 30 points per week.
• Weekend or on-call requirement: none.
• Coverage areas include Woodford, Mercer, and Anderson counties.
