About the Role:

• Hosptial Liaison will serve as the primary point of contact between the community and healthcare providers, ensuring prompt and accurate communication regarding resident care needs and treatment plans.

• Collaborate with healthcare providers to coordinate medical appointments, medication management, and other healthcare-related services.

• Develop and maintain positive relationships with healthcare providers, ensuring consistent communication and cooperation to ensure the best possible outcomes for our residents.

• Hospital Liaison will also assist with the coordination of resident discharges, transfers, and admissions, ensuring a smooth transition for our residents.

Responsibilities:

• Hospital Liaison will maintain accurate and timely records of resident care, treatment plans, and communication with healthcare providers.

• Coordinate and facilitate resident care conferences and meetings with healthcare providers.

• Participate in quality improvement initiatives to ensure the highest level of care for our residents.

• Hospital Liaison will also provide educational support and resources to residents and their families, empowering them to take an active role in their healthcare.
