Interim HealthCare of Chicagoland is seeking an experienced RN Hospital Liaison to partner with one of Chicago's leading hospital systems. In this highly visible position, you'll work alongside physicians, case managers, discharge planners, patients, and families to ensure safe, seamless transitions from the hospital to home health care.

Every day, you'll help patients receive the care they need in the comfort of their own homes while reducing unnecessary hospital readmissions and improving patient outcomes.

What You'll Do

As our RN Hospital Liaison, you will serve as the clinical bridge between the hospital and home, ensuring patients experience a safe and successful transition of care.

Your responsibilities include:

• Collaborate with physicians, discharge planners, case managers, and hospital staff to coordinate home health services.
• Identify patients who would benefit from skilled home health care.
• Assess patients' clinical and home care needs prior to discharge.
• Meet with patients and families at the bedside to educate them about Interim HealthCare's services and answer questions.
• Coordinate complex patient transitions to promote continuity of care and reduce avoidable readmissions.
• Review referrals, obtain essential clinical information, and communicate with our Intake and Clinical Management teams.
• Assist with insurance eligibility, authorizations, and physician orders when appropriate.
• Build strong relationships with hospital staff while serving as a trusted clinical resource.
• Participate in quality improvement initiatives and collaborate with interdisciplinary teams to improve patient outcomes.

What You'll Bring

Qualifications

• Current Illinois Registered Nurse (RN) license
• Minimum of two years of strong clinical nursing experience
• Home Health, Home Care Intake, Case Management, Care Coordination, or Hospital Discharge Planning experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and relationship-building abilities
• Comfortable working with EMR systems, Microsoft Excel, and healthcare software
• Reliable transportation for travel between assigned facilities
