Job Description
Title: Home Health RN

Schedule: Tuesday to Saturday

Assignment Details:
Guaranteed Hours: 36
Contract Weeks: 16

Requirements:
- Current Nursing License
- CPR Certification
- Experience in home health or hospice care preferred

Roles & Responsibilities:
- Provide individualized, goal-oriented nursing care according to physician's orders.
- Serve as Case Manager for patients receiving care, leading interdisciplinary teams when necessary.
- Monitor and document patient progress and adjust care plans as needed.
- Educate patients and families on health management and care options.
- Coordinate with healthcare providers to ensure comprehensive patient care.
- Advocate for the patient's needs and preferences throughout the care process.
