Leaders are also expected to work in a manner that demonstrates a commitment to quality, patient safety, employee engagement, innovation, and model for all other employees the highest standards of personal integrity, professionalism, and competence.OUR CORE VALUESCompassionExcellenceIntegrityLearningRespectSustainabilityQualificationsESSENTIAL SKILLS AND EXPERIENCE:High school diploma or GED from an accredited institution is required, Associate's Degree preferredMinimum of one (1) year of experience working in the Behavioral Health field preferredExcellent customer service skillsExcellent computer skillsThorough knowledge of current versions of DSM and ICD, preferredAbility to complete a Behavioral Health Assessment with appropriate clinical oversightAbility to provide effective and respectful quality care and services that are responsive to diverse cultural beliefs and practicesCertification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red CrossBilingual English/Spanish strongly preferredValid Arizona driver's licenseValid Level One Fingerprint Clearance Card issued by Arizona Department of Public SafetyPOSITION RESPONSIBILITIES/ESSENTIAL:Participate as directed in the screening of new service requests within the center ensuring that all individuals are adequately and appropriately served according to their individual needs and in accordance with existing policies and proceduresParticipate in regular interdisciplinary staff meetingsAccept newly assigned cases in which the diagnostic intake has been completed by the Intake Specialist. Completes case management assessment on all clients entering caseloadDevelop appropriate treatment/service plans with clients as the means for implementing appropriate service and developing effective alliances with clientsProvide direct client services to individuals and/or family members. Services provided may include case management, skills training, group skills training, health promotion, transportation, supportive employment services, counseling with appropriate clinical oversight and others depending on client need. Services may be provided in office, home, school or community settingsProvide on-going supportive and/or case management functions in accordance with the problems, needs and the strategies identified within the service plan in order to assist the client to achieve goals and objectivesProvide face to face reviews with the client and/or relevant behavioral health staff on a regular basis. Ensures progress made is reported so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved. The frequency reviews will be determined by relevant requirementsAssist supervisory staff with tracking of clients, including the generation of reports that will allow follow-through on quality of careDocument all service contacts on a timely basis including face-to-face interviews, collateral and networking contacts, correspondence and maintains the case records in accordance with internal and regulatory standards requirementsMaintain close communication with the consulting psychiatrist and the Behavioral Health team for input regarding medication compliance, side effects of medication, and medication changes and alerts behavioral health team of any changes in client adjustment which might suggest decompensation and a need for more aggressive interventionPerform other duties as assigned by the supervisor which are consistent with the position and in compliance with Adelante Health Care policies and proceduresNONESSENTIAL SKILLS AND EXPERIENCE:Experience with community resources and agencies, preferredAdditional Duties and ResponsibilitiesOther duties as requested or assignedPCMHAll employees are responsible for promoting and participating in interdisciplinary communication and collaboration to enhance the delivery of quality across the health care system.
