Job Summary

We are seeking a skilled and dedicated Evening Shift LPN to join our interdisciplinary care team at Bradford Hills Nursing & Rehabilitation Center. As an LPN on our evening shift, you will play a vital role in ensuring the health and well-being of our residents and patients. You will provide high-quality care, perform vital signs, administer medications, and assist with daily living activities. Responsibilities

• Provide direct patient care, including vital sign monitoring, medication administration, and personal care assistance

• Collaborate with the interdisciplinary care team to develop and implement individualized care plans

• Conduct daily rounds to assess resident and patient needs, observing and reporting any changes or concerns

• Assist with daily living activities, such as bathing, dressing, and grooming

• Document patient information accurately and in a timely manner

• Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements

• LPN certification

• Strong communication and interpersonal skills

• Ability to work effectively as a team member

• Ability to prioritize and manage multiple tasks simultaneously

• Strong attention to detail and ability to maintain accurate records
