Rn, Community Health Nurse (full-time)

Avera Sacred Heart Hospital - Registered Nurse (RN) Work Shift: Primarily daytime with night, weekend and holiday on‑call involvement (United States of America). Shift Information Three 8‑hour shifts and one 12‑hour shift per week. Weekday call once a week/every other week. Every 10th weekend on call. One holiday per year on call. Student Loan Repayment This position may be eligible for Avera’s Student Loan Repayment Program. Each month an extra payment of $167 (up to $10,000) will be contributed by Avera to help pay down the principal balance faster. Responsibilities Develop nursing diagnosis, establish nursing care objectives, and define nursing interventions. Provide physical, emotional, psychological, and spiritual support to patients, families, and friends to understand patient conditions, medications, and self‑care skills. Review physician prescriptions and communicate the nursing care plan and pertinent information to the healthcare team. Coordinate with other healthcare providers and conferring with physicians. Obtain medical histories and document patient physical condition, psychological status, and influencing social factors. Teach healthcare requirements by establishing patient care goals and instructing patients and families on condition management. Evaluate patient care by adhering to therapeutic standards, measuring health outcomes, and re‑evaluating care objectives; modify nursing care plans accordingly. Maintain a safe, secure, and healthy patient care environment by adhering to infection‑control policies and protocols, medication administration procedures, and controlled substance regulations. Document patient care services through charting in patient and department records. Operate and maintain patient care equipment by following operating instructions and established procedures. Keep patient care supplies ready by checking stock and stocking items. Maintain competence in nursing capabilities as outlined by leadership. Qualifications Active Registered Nurse (RN) license in the state of practice upon hire. Ability to perform each essential job function satisfactorily, including adequate visual acuity and effective communication skills.
