Director of Nursing (DON) - Home Health (Hybrid)

Location: Bloomington, MN
Job Type: Full-Time (Hybrid)
Schedule: Monday-Friday | 8:00 AM - 4:30 PM
On-Call: Shared Rotation

Job Summary

We are seeking an experienced Director of Nursing (DON) to lead and oversee clinical operations for a home health agency. This hybrid leadership role combines office-based management with occasional patient home visits, ensuring high-quality clinical care, regulatory compliance, and operational excellence. The ideal candidate is a strong clinical leader with home health experience, exceptional organizational skills, and a commitment to improving patient outcomes.

Key Responsibilities

• Provide leadership and oversight for all clinical nursing operations.

• Supervise, mentor, and support nursing, office, and field staff.

• Ensure compliance with Medicare Conditions of Participation, state, federal, and agency regulations.

• Oversee OASIS documentation, clinical quality, and regulatory compliance.

• Coordinate interdisciplinary patient care to achieve positive clinical outcomes.

• Recruit, onboard, train, and evaluate nursing staff.

• Conduct performance evaluations, coaching, and quality improvement initiatives.

• Investigate and resolve patient care concerns and complaints.

• Develop and maintain clinical policies, procedures, and standards of practice.

• Collaborate with physicians, referral sources, and community partners.

• Participate in strategic planning and continuous performance improvement initiatives.

• Perform occasional patient home visits to remain engaged in clinical care (typically one day per week or two half-days as needed).

Qualifications

• Active Registered Nurse (RN) license.

• Minimum of 2 years of RN experience .

• Minimum of 2 years of supervisory or management experience .

• Previous Home Health experience is required.

• Strong knowledge of OASIS documentation and Medicare Home Health regulations .

• Experience working with Medicare, VA, and other insurance providers .

• Experience using Epic EMR .

• Excellent leadership, communication, organizational, and problem-solving skills.
