Description

• Conduct evaluations of patients in both inpatient and outpatient settings while crafting personalized treatment plans
• Administer physical therapy interventions that prioritize mobility, strength, balance, and overall function
• Collaborate closely with nurses, physicians, case managers, and fellow rehabilitation professionals to enhance patient outcomes
• Provide education to patients and their families on therapeutic objectives, discharge planning, and appropriate home exercises
• Maintain comprehensive documentation of evaluations, treatments, and patient progress in alignment with facility standards and regulations
• Engage in care team discussions and interdisciplinary meetings, ensuring coordinated and effective patient care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialty certifications (such as neurological or geriatric) are welcomed but not essential

Skills

• Prior experience in a clinical setting is preferred
