Description

• Conduct thorough evaluations of patients in inpatient or outpatient settings to develop tailored treatment plans
• Implement physical therapy interventions focusing on mobility, strength, balance, and overall function
• Collaborate with nursing staff, physicians, case managers, and other rehabilitation experts to enhance patient outcomes
• Educate patients and their families on therapeutic objectives, discharge planning, and home exercise recommendations
• Accurately document evaluations, treatments, and patient progress in alignment with regulatory and facility protocols
• Engage in care team rounds and multidisciplinary conferences to ensure cohesive care and support

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized program

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (such as neurology or geriatrics) are advantageous but not mandatory

Skills

• A proven track record of previous experience is highly preferred
