Housing & Care Coordinator for Homelessness Support

Community Bridges, Inc. (CBI) seeks a Client Care Coordinator to manage housing caseloads and coordinate care for individuals/families experiencing homelessness. You will connect clients to PCMH/PCP services and monitor community-based services across the continuum of care. The role requires coordinating with RBHA and non-RBHA providers, documenting outcomes, and supporting clients with complex housing, health, and behavioral health needs. #J-18808-Ljbffr Community Bridges, Inc.
