POSITION SUMMARY: Personal Care Workers is responsible for providing and assisting participants with activities of daily living and supportive care necessary to the preservation of the home environment. Assists in monitoring the health status and providing a safe environment for participants. Observes participants taking prescribed medications. Reports pertinent information to Home Care Coordinator in a timely mannerAssists participants with IADLs (grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care) as assignedDocuments services provided to participants consistently and accurately via Electronic Health Record (EHR). Follows procedure for flow sheet completion. Documentation is completed in a timely mannerParticipates in participants care planning through cooperation with multidisciplinary teamsHandles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautionsParticipates in Company required and assigned training programs and any required staff and training meetingsOther duties as requested/assignedHigh school diploma or equivalentMinimum of one year of experience with a frail or elderly population preferredMust have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customersMust have a current Colorado Drivers License and driving record must be in compliance with Company policy.
