Type: Field role Type of Hire: Direct Placement Hiring Decision: Made by client agency Available Openings: 2 Responsibilities Conduct detailed assessments of patients and their families. Develop and manage individualized care plans. Coordinate with interdisciplinary teams to provide holistic care. Regularly review and update care plans based on patient needs. Provide emotional support and education to families and caregivers. Qualifications Certification: Licensed Registered Nurse with no restrictions. Experience: Minimum 2 years in hospice care.
