Community Health WorkerUnder the direction of the Manager/Director, the Community Health Worker (CHW) will serve as a trusted liaison between enrollees/members, the healthcare system, and community resources. The CHW will conduct outreach and, within the established scope of practice, home and community visits to engage members, build sustainable relationships, and help reduce barriers to care.The CHW will collaborate closely with the Case Management team to support enrollees/members in accessing appropriate healthcare services, addressing social determinants of health (SDoH), navigating community resources, and following through with recommended care. Through culturally responsive education, advocacy, and care navigation, CHW will help to improve member engagement, optimize the use of healthcare resources, and enhance the overall member experience.Essential Duties and ResponsibilitiesAccording to the ADA 1990, reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Supports the achievement of departmental goals and objectives and complies with organizational policies, procedures, quality standards, safety requirements, and applicable governmental and accreditation regulations.Conducts outreach to members by telephone and as assigned, through home or community visits in collaboration with the Case Management team and within the CHW scope of practice.Builds trusting, culturally responsive relationships with members to promote engagement in healthcare and community services.Provides health education and advocacy through individual interactions, group education, workshops, and community-based activities to promote preventive health practices and address barriers to care.Collaborates with Case Managers to engage members identified as having frequent emergency department visits, hospitalizations, or other utilization patterns that may benefit from care management support.Assists members with navigating healthcare and community resources, including primary care, specialty care, behavioral health, transportation, food, housing, and social service programs.Supports members in overcoming barriers to care by assisting with appointment scheduling, transportation coordination, follow-up after healthcare visits, and addressing identified SDoH needs.Reinforces the member's care plan and encourages adherence to recommended services and follow-up, while referring clinical questions or concerns to the appropriate licensed healthcare professional.Participates in community outreach events, health fairs, and other activities that promote member engagement and community wellness.Documents all outreach, member interactions, referrals, and activities accurately and timely in the electronic medical record and other required systems.Participates in required training, team meetings, case conferences, and educational opportunities and applies new knowledge to daily work.Maintains appropriate professional boundaries, protects member confidentiality, and performs all duties within the defined Community Health Worker scope of practice. The CHW does not perform clinical assessments, develop clinical care plans independently, or make clinical decisions.Perform other duties as assigned.Comply with all policies and standards.Utilizes all organizational resources effectively and efficiently.Where applicable, refer to the competency validation documents for population(s) served.QualificationsHigh School Diploma or GED is required.Possess meaningful experience with community organizations, verified by personal and professional references with personal knowledge of this fact.Possess and maintain a driver's license with a motor vehicles administration driving record that is free of accidents, moving violations, or points.Ability to comprehend and follow the policies and procedures for this role.Ability to read, write, and speak or communicate in English to successfully accomplish the essential duties of the position.Ability to demonstrate ethical behavior that supports the organization's mission, values, and commitment to compliance with all federal, state, and regulatory laws.Inspires trust and exhibits honesty and integrity within the scope of daily activities.Exhibits professionalism, courtesy, and excellent customer service while interacting with patients, guests, and co-workers.Ability to work effectively with people from a variety of culturally diverse backgrounds.Ability to maintain patient confidentiality.Ability to hear and communicate orally.Work EnvironmentThe working environment noise level is moderate.
