Overview Exempt | Driving Required PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Housing Navigation Team as the Case Manager/Housing Navigator at the North Long Beach Office . ABOUT PATH - Since its foundation, PATH (People Assisting The Homeless) has pioneered bold and effective approaches to assist people experiencing homelessness. PATH operates services throughout California, connecting clients to a continuum of homelessness prevention, street outreach, employment preparation and placement, individualized case management, supportive services, interim housing, and permanent supportive housing. ABOUT OUR TEAM - Our programs holistically address barriers to long-term housing stability, implementing evidenced-based practices to promote wellbeing beyond housing. About the Job As part of the Housing Navigation Team , the Case Manager/Housing Navigator helps clients break the cycle of homelessness using a “whatever it takes” approach. Position Responsibilities Conduct intake and individualized needs assessments for all clients and collaborate to develop an Individualized Service Plan (ISP) addressing housing goals, income goals, medical, mental health, substance use, financial resources, vocational, and social support needs. Provide referrals, linkages, information, and support to resources that help clients achieve their ISP goals. Complete program-specific assessments (e.g., VI-SPDAT). Evaluate strengths and challenges to address short-term and long-term goals; conduct a check-in after initial intake. Maintain documentation standards as required by the Program and PATH policies; complete case file notes in a timely manner and maintain a complete, accurate, and current client file with all required documents. Maintain confidentiality of client files per HIPAA and applicable guidelines. What You Bring We’re looking for candidates with: Analytical Skills and Attention to Detail: Gather and interpret data, identifying trends to inform decision-making; ensure high-quality, accurate work. Collaboration and Teamwork: Work effectively with colleagues, participants, and stakeholders; contribute to team goals. Communication: Convey information clearly in writing and verbally. Creativity and Initiative: Take proactive action and generate innovative solutions. Critical Thinking and Problem Solving: Analyze information, identify key issues, consider alternatives, and make sound decisions. Ethics and Boundaries: Maintain ethical standards, professionalism, and boundaries. Preferred Qualifications Two (2) years of experience in healthcare, case management, homeless services, or related fields. Minimum Qualifications All levels of education and experience are welcome. Mandatory Requirements Regardless of qualifications, candidates must: Have employment eligibility verification Have or be able and willing to obtain CPR/First Aid training Successfully complete: Tuberculosis Test, Background Screening, Drug Test Driving Driving is an essential function of this position. To meet employment requirements, all candidates must: Have reliable transportation Have a valid driver’s license Provide proof of insurance and ownership for personal vehicles used during work duties Be able to qualify for PATH's insurance coverage Employment Eligibility Applicants must be authorized to work for any employer in the U.S. PATH does not sponsor employment visas. California-Based Work Expectations We are a California employer. Regular work must be performed within the state. Work outside California requires approval and does not imply remote work. PATH does not offer fully remote positions. Some roles may involve multiple worksites or approved off-site locations, but many positions are fully on-site. Schedules may be adjusted based on responsibilities and operational requirements.
