Provides community outreach, engagement, and crisis intervention services, consults with clinical staff when appropriate, completes assigned county processes, and promotes destigmatizing mental health education. Delivers services in homes, schools, workplaces, community, and office settings, and documents all services accurately and timely in the Electronic Health Record.Division/Program Overview:Population Served (Capacity): Serves clients (ages 16-24).Facility & Setting: Outpatient community-based services and support. Delivers client services throughout Fresno County within designated county limits.
