Position Summary
We are seeking an outgoing, relationship-driven Hospice Sales & Community Outreach Representative.
The ideal candidate is a motivated healthcare sales professional who already has established relationships with hospital case managers, physicians, physician practices, discharge planners, social workers, skilled nursing facilities, assisted living communities, and other healthcare referral sources.
Previous hospice or home health experience is highly preferred but not required. Candidates with strong healthcare relationships and a demonstrated ability to develop referral business will also be considered.
This is a highly visible, field-based position requiring consistent face-to-face interaction with healthcare professionals, referral partners, patients, families, and community organizations.
Key Responsibilities
Referral & Business Development

• Develop and maintain strong relationships with hospital case managers, physicians, discharge planners, social workers, healthcare administrators, and other referral partners.
• Generate qualified hospice referrals and convert relationships into new patient admissions.
• Build new referral relationships while expanding existing accounts.
• Educate healthcare professionals and community partners about hospice services, eligibility, benefits, and the referral process.
• Develop relationships with hospitals, physician practices, skilled nursing facilities, assisted living facilities, rehabilitation facilities, and other healthcare organizations.
• Identify new referral opportunities and develop strategies for increasing hospice census.
• Maintain consistent communication and follow-up with referral sources.
• Understand and appropriately communicate hospice services available to patients with Medicare, Medicaid, commercial insurance, and other applicable coverage.

Field Marketing & Account Management

• Maintain an active field presence, typically completing 8–10 referral-source visits per day.
• Plan daily routes and prioritize accounts based on referral potential and business-development objectives.
• Accurately document and verify daily visits, contacts, follow-up activities, referrals, and outcomes.
• Maintain an organized pipeline of prospective and active referral sources.
• Track referral activity and progress toward monthly admission goals.
• Follow up promptly on potential referrals and coordinate with the clinical/admissions team to support timely patient evaluations and admissions.
• Provide management with regular updates regarding referral activity, market intelligence, competitive activity, and business-development opportunities.

Community Outreach

• Represent the organization at community events, healthcare events, networking functions, senior-focused programs, educational events, and other outreach activities.
• Develop partnerships with organizations that serve seniors, caregivers, patients, and families.
• Identify opportunities to increase the organization’s visibility within the community.
• Help coordinate and participate in community outreach initiatives.
• Build relationships beyond traditional healthcare referral sources to establish the organization as a trusted community resource.
• Participate in approximately 1–2 weekend events or outreach activities per month, with corresponding time off provided during the workweek.

Qualifications
Preferred Experience

• Healthcare sales, business development, physician relations, community outreach, hospice marketing, home health marketing, or related experience
• Existing relationships with hospital case managers and physicians strongly preferred
• Established healthcare referral network highly desirable
• Hospice, home health, senior care, skilled nursing, assisted living, DME, rehabilitation, or related healthcare experience is a plus
• Demonstrated ability to generate referrals and develop productive professional relationships
• Experience working toward measurable sales or census-growth goals

Schedule & Work Environment

• Primarily field-based healthcare sales and community outreach
• Regular weekday business hours with flexibility based on referral-source availability and community activities
• Approximately 1–2 weekends per month may be required for community outreach and business-development events
• Equivalent weekday time off will be provided when weekend work is required
• Local travel throughout the assigned territory is required
• Business casual professional attire is expected when representing the organization
