Case Manager: Family Services & Community Support

Oaks Integrated Care

Oaks Integrated Care in Moorestown Township is hiring a Case Manager to support families in obtaining housing and community resources. You will engage in outreach, support, and advocacy across various life domains, and maintain collaborative relationships with service partners. Eligible candidates will hold a Bachelor's degree in social work or psychology and a valid driver's license.
